How to Apply

Applications are accepted on an annual basis from September 1 through December 1. Top candidates will be invited for an interview at Memorial Hermann between January and February.

Application Requirements

  1. Physical therapy degree from an accredited physical therapy educational program
  2. Licensed or eligible for licensure in the state of Texas
  3. APTA member (membership must be maintained throughout residency)
  4. Current certified athletic trainer designation, licensed emergency medical technician, or certification as an emergency responder (Sports Residency ONLY)

To apply, please print out and complete the appropriate application documents below and fax or mail to the address specified in the Application Packet Checklist.

Please include the following documents:

  • Letter of Intent (no more than 2 pages)
  • Curriculum Vitae/Resume
  • Three Letters of Recommendation:
  • New graduates: Please submit at least one recommendation from a professor and one recommendation from a clinical instructor.


All applications are due by December 1st to be considered for enrollment the following calendar year. A non-refundable $50 application fee must be submitted with your application.
