Palliative care is not Hospice. Palliative care is focused on providing patients relief from the symptoms, pain, and stress of serious illness - regardless of the diagnosis, aggressiveness of treatment, or expected outcome of the illness.
The nurses help patients understand their illness and provide the specialized care and support needed to improve the quality of life for the patient and their families. Palliative care is provided in the patient's home by a team that includes a Nurse Practitioner, Palliative Certified Nurses, and other specialists who work with the patient's doctors to provide an extra layer of support. By managing the patient's symptoms we can help them remain at home and avoid hospitalizations.
* A physician's order and statement confirming it is a taxing effort for the patient to leave the home is required to provide this program.
