Cardiac Life is a specialized medical program for people with cardiac disease.
Nurses will teach the patient how to manage their disease and identify symptoms requiring medical attention. The focus is on providing relief from the symptoms, pain, and stress of a serious illness. Care is provided in the patient's home by a team that includes a Nurse Practitioner, Cardiac trained Nurses, and other specialists who work with the patient's doctors to provide an extra layer of support. By managing symptoms, cardiac patients on this program can avoid hospitalization and remain at home.
* A physician's order and statement confirming it is a taxing effort for the patient to leave the home is required to provide this program.
