If you have test results or medical records that are not from The University of Texas Medical School at Houston or Children’s Memorial Hermann Hospital, please obtain them prior to your first visit and mail them to:

Getting Insurance Approval
In order to get your health insurance provider to cover your visit to the Neurometabolic and Mitochondrial Clinic:
- Call your health insurance provider and explain the reason for the visit.
- Ask your insurance company if Children’s Memorial Hermann Hospital and physicians at The University of Texas Medical School are in-network providers.
- Verify coverage and whether or not a referral from your child's primary care physician is needed. If a referral is needed from your PCP, please have them fax it to:
