Patients referred to our center undergo state-of-the-art diagnostic evaluation using the most recent technologies and receive individualized and compassionate medical management. The Home Management Plan of Care was developed as an educational tool to assist families in being proactive in controlling asthma exacerbations and triggers. The action plan form was designed specifically for the primary care provider to use with the family to manage mild to moderate asthma symptoms. Families should use this form to guide discussions with their primary pediatrician or family physician. Use of the asthma action plan offers a collaborative, standard approach to the management of asthma symptoms. Consistently using the same asthma action plan format enhances understanding and prevents confusion.
