Diabetes Self Management Program
Memorial Hermann Greater Heights Hospital has been Recognized by the American Diabetes Association for meeting the National Quality Standards for Diabetes Self-Management Education.
Our program is for people with all types of diabetes, including type 1 diabetes, type 2 diabetes and gestational diabetes, and both newly diagnosed and well-established patients that want to improve their blood glucose control.
An individualized approach, which may include a group learning session, allows the patient to identify the behavior changes and strategies needed to control diabetes. The Diabetes Education Program is staffed by nurses and dietitians specializing in diabetes management.

Make an Appointment
A physician’s referral may be required by the patient’s insurance company for diabetes training to be covered by insurance. Medicare requires a referral, as well. Most insurance providers cover the costs of the program, and a co-pay may apply.
To schedule an appointment for diabetes education, please call (713) 867-3336 Monday through Friday from 8 a.m. until 5 p.m. Patients are welcome and encouraged to bring family members to appointments.
Diabetes Support Group in Northwest Houston
Memorial Hermann Greater Heights Hospital hosts a Diabetes Support Group the last Tuesday of every other month from 4:30 to 6 p.m. in the South Tower Classrooms located on the first floor of the Memorial Hermann Greater Heights Hospital, 1635 North Loop West.
