Diabetes Self-Management Education Program
Living well with diabetes is as individual as you are. That is why The Diabetes Center at Memorial Hermann Memorial City Hospital offers a program that begins with a private one-on-one session with a Certified Diabetes Educator. You will receive an assessment of your diabetes needs as well as tools to help you achieve your personal health goals. After your first appointment, we offer you continued sessions based on your specific needs. You will find that our appointment times have been designed to meet a wide variety of personal schedules.
Our Self-Management Program is recognized by the American Diabetes Association; an organization that sets the gold standard for excellent diabetes care through education, experience and credentialing. The focus of the program is based on your individual needs and personal goals and may include one or all of the following:
- Healthy eating
- Being active
- Glucose monitoring
- Problem solving
- Risk reduction
- Healthy coping
Translation services are available for any language. Videos and printed materials are also available for a variety of different languages.
