Diabetes Self-Management Program

Though diabetes is a serious condition, patients find it manageable at home in most circumstances if they are properly trained and commit to lifestyle changes that may be required.

Training available at Memorial Hermann Memorial City Medical Center's Diabetes Center is an important first step in making self-management possible. Our program, which has been recognized by the American Diabetes Association, provides the information, tools and techniques patients need for the daily management of their disease.

The Program

Diabetes self-management training classes consist of a 4-hour initial session in a small group, where the basics of management are taught, including:

  • Diabetes facts
  • Complications
  • Medications
  • Basics of food management
  • Glucose monitoring

Three subsequent follow up sessions, held weekdays, occur approximately one month, three months, and six months after the initial class. These one-on-one sessions last an hour each, and those who attend the follow-ups show a much higher management success rate.

Class dates and times vary. Initial classes are held Tuesdays, Wednesdays or Thursdays. Classes may be held at either 1-5 PM or 4-8 PM at the Diabetes Center, 3rd Floor, Memorial Hermann Memorial City Medical Center. Follow-ups are scheduled individually Monday through Friday. A Saturday class is held once a quarter from 1-5 PM.

Instructors

Only certified educators teach in our program, which is recognized by the American Diabetes Association.

To become certified, an instructor must complete 1,000 hours of patient education, have a bachelor of science in nursing degree or be a registered dietitian, and pass the National Certification Board for Diabetes Educators' exam.

A Spanish-speaking diabetes educator is available. Videos and printed information in Spanish are also available.

A physician's referral may be required by the patient's insurance company for diabetes training to be covered by insurance. Medicare requires a referral, as well. Most insurance providers cover the costs according to policy restrictions or limitations, and a co-pay may apply.
