2016 Community Health Needs Assessment
As a part of providing Community Benefit to enhance the health and well-being of the residents of the southwest area, Memorial Hermann Southwest Hospital is required by the Patient Protection and Affordable Care Act to identify the community’s health needs and strengths by conducting a Community Health Needs Assessment (CHNA) to prioritize health issues, to provide a foundation for the development of a community health improvement plan, and to assist community organizations in their program planning.
Guided by a collaborative approach where health was examined in its broadest sense, the most recent CHNA was conducted between August 2015 to March 2016 with the overall goal of identifying the major health care issues, barriers to access, and health priorities for those living in the Houston community. The analysis included a review of secondary data on social, economic and health issues in the region with qualitative information from focus groups with community residents and service providers and input from numerous community representatives.
During the assessment, the following three Health Priorities were identified for the southwest community:
- HEALTHY LIVING
Issues: Early detection and screening, overweight/obesity prevention, nutrition education, access to healthy food, importance of making time for/safety during physical activity, and chronic disease management
- BEHAVIORAL HEALTH
Issues: Access to and availability of psychiatrists and mental health providers, stigma, quality of mental health and substance abuse services
- HEALTH CARE ACCESS
Issues: Availability of primary care and specialty providers, health insurance coverage and costs, transportation, health care navigation to appropriate level of services
The CHNA process culminated in the development of a Strategic Implementation Plan (SIP) for each hospital to address their community’s significant needs identified through the CHNA and to align with a core set of Memorial Hermann Health System priorities. Memorial Hermann Southwest Hospital will utilize the plan as a guide over the next three years to support the health of their community and advance the mission of the organization.

Memorial Hermann Southwest Hospital CHNA and community benefit Implementation Plan were reviewed and approved by the Community Relations Committee of the Health System Board in June 2016 and September 2016 respectively.
