Community Health Needs Assessment Southwest
As a part of providing Community Benefit to enhance the health and well-being of the residents of the greater Houston area, Memorial Hermann Southwest Hospital is required by the Patient Protection and Affordable Care Act to identify the community’s health needs by conducting a Community Health Needs Assessment (CHNA) to prioritize health issues, provide a foundation for the development of a community health improvement plan, and assist community organizations in their program planning. The CHNA was conducted between August 2012 to February 2013 with the overall goal of identifying the major healthcare needs, barriers to access, and health priorities for those living in the southwest Houston community. The analysis included a review of current health data, demographics and input from numerous community representatives.
During the assessment, the following six Health Priorities were identified for the southwest Houston community:
The process culminated in the development of an Implementation Plan to address the significant needs identified through the CHNA. Memorial Hermann Southwest Hospital will utilize the plan as a guide over the next three years to support the health of their community and advance the mission of the organization.

Memorial Hermann Southwest Hospital’s CHNA and community benefit Implementation Plan were reviewed and approved by the Community Relations Committee of the Health System Board in June 2013.
