Diabetes Self-Management Program
The goal of the Diabetes Self-Management Program is to provide patients with the knowledge, skills and motivation needed in order to effectively self-manage their diabetes. Effective self-management helps to minimize, delay or avoid complications associated with the long-term disease process.
This program is for people with all types of diabetes: type 1 diabetes, type 2 diabetes, gestational diabetes, newly diagnosed and well-established patients that want to improve their blood glucose control.
An individualized approach, that may include a group setting, allows the patient to identify the behavior changes needed to control diabetes. Diabetes Education Services is staffed by instructors specializing in diabetes management.

A Physician’s Referral is required. Instruction in other languages available upon request.
