Geriatric Care Unit
Memorial Hermann Southwest Hospital is now providing a dedicated unit for Geriatric patients. Older adults, especially those that are acutely ill, need special care. The Geriatric Care Unit has an interdisciplinary team providing patient centered care by focusing on preventing complications and promoting physical functioning and independence.
The team includes a board certified geriatrician medical director, a geriatric nurse practitioner, geriatric trained professional nursing staff, a social worker, a case manager, a therapist (physical, occupational and speech), a pharmacist, a nutritionist and pastoral care. Team members are educated through the Hartford Institute for Geriatric Nursing, which identifies and develops best practices in nursing care for older adults.
Maintain a patient-centered program that specializes in care for the elder to include:
Environmental modifications to promote independence
• Interdisciplinary team approach to care
• Pharmacist-supervised review of medication profile
• Nursing care delivered using evidence-based geriatric protocols
• Family friendly atmosphere
Minimize functional decline in elders by:
- Early evaluations by Physical Therapy and combined therapy during hospital stay
- Reduction in the use of restraints
- Use of protocols to reduce falls and iatrogenic complications
Increase coordination of care through interdisciplinary team:
- Reducing avoidable discharge delays
- Timely referrals to appropriate disciplines
- Communication with referring facility to enhance seamless transition and continuity of care
