HomeSight is a program designed for patients with Poor Vision.
A nurse will come to the patient's home to evaluate how the patient's vision has affected their independence and safety in their home. An occupational therapist will provide training to regain their independence and safety in their home. The Home Sight Therapist will assist in creating new tools that will help the patient complete simple tasks that have become difficult due to their low vision.
* A physician's order and statement confirming it is a taxing effort for the patient to leave the home is required to provide this program.
