TIRR Memorial Hermann Outpatient Rehabilitation at Northwest offers individualized single or multidisciplinary therapy for adults and for children ages 12 and above. We provide rehabilitation services for individuals with:

Admissions and Referrals
Admission to Outpatient Rehabilitation requires a prescription from a referring physician. We welcome self-referrals as well as referrals from family members, case managers, insurance companies and healthcare professionals.
