Hermann Home Health Receives Award for Quality Improvement Initiative
Houston, Texas, May 25, 2005 – Memorial
Hermann Home Health recently received recognition for its continued
dedication to quality patient
care. The
distinction came as a result of effective collaboration with the
Texas Medical Foundation (TMF).
Memorial Hermann
Home Health enables patients
to continue living in their own homes. Staff members coordinate and
provide skilled care and therapy, directed
by doctors, with the goal of helping patients regain independence and self-sufficiency.
Memorial
Hermann Home Health was one of the first organizations in Texas
to engage in a voluntary process through which the TMF helps healthcare
agencies
improve
results in specific clinical areas. Memorial Hermann excelled in the two
criteria
selected – managing oral medications and transferring patients (from
bed to chair, for example).
To measure and
encourage such improvements, the national Centers for Medicare & Medicaid
Services launched a Home Health Quality Initiative in October 2003. The
program provides comparative results through the Home Health Compare website.
The most
recent data shows Memorial Hermann Home Health ahead of the state average
in nine of the 10 quality measures reported.
