Release of Medical Records
The Memorial Hermann Release of Information Department is dedicated to processing your requests for patient-protected health information in a timely manner, while maintaining patient confidentiality our top priority. Hours of operation are Monday through Friday, 8 a.m. to 4:30 p.m. For your convenience, we have one mailing address for all Memorial Hermann hospitals and outpatient centers. To request your medical record, please complete the authorization form and mail back to the mailing address below.

If you need your records for medical care and would like for us to fax records to your physician, you may fax the completed authorization form to our office for processing. Facility information is listed below. Please make sure to include the doctor’s phone number, as we must confirm their number before faxing your medical records.

Yes, there is a fee for copies of medical records. The fees are set by the Texas Health and Safety Code 241.154(e) and updated annually to reflect the most recent changes to the consumer price index as published by the Bureau of Labor Statistics.

There is no fee for viewing your own medical records. An authorization form must be completed and sent to Release of Information Department. Because space is limited, appointments are scheduled for a maximum of 2 hours at a time. Please call the hospital Release of Information Department to make an appointment.
Medical records are the hospital’s documentation of a patient’s medical care and services provided. They are the property of MHHS.
