Outpatient Rehabilitation offers individualized single or multidisciplinary therapy for adults and children.

Outpatient Rehabilitation Team

Depending on individual needs and customized treatment plans, the outpatient rehabilitation team may consist of:

  • The patient and their family/friends
  • Affiliated physicians
  • Physical therapists
  • Occupational therapists
  • Speech and language pathologists
  • Neuropsychologists
  • Licensed clinical social workers
  • Vocational counselors

Admission & Referrals

Admission to outpatient rehabilitation requires a prescription from a referring physician. We welcome self-referrals as well as referrals from family members, case managers, insurance companies and healthcare professionals for patients who need care in one of the following areas:
