Outpatient Rehabilitation offers individualized single or multidisciplinary therapy for adults and children.
Outpatient Rehabilitation Team
Depending on individual needs and customized treatment plans, the
outpatient rehabilitation team may consist of:
- The patient and their family/friends
- Affiliated physicians
- Physical therapists
- Occupational therapists
- Speech and language pathologists
- Neuropsychologists
- Licensed clinical social workers
- Vocational counselors
Rehabilitation Goals
Rehabilitation goals may include:
- Increased level of independence and mobility
- Increased independence with activities of daily living skills
- Decreased fall risk
- Improved postural control and stability
- Reduced visual deficits
- Improved swallowing (dysphagia)
- Improved voice production
- Improved cognitive function
- Return to school
- Return to work
Admission & Referrals
Admission to outpatient rehabilitation requires a prescription
from a referring physician. We welcome self-referrals as well as referrals from
family members, case managers, insurance companies and healthcare professionals
for patients who need care in one of the following areas:
