The Hospice Team

Physician-directed services by the hospice medical director are offered in conjunction with the patient's primary physician. The hospice medical director will make home visits as needed.

Registered nurses are highly skilled in pain management and symptom control, coordinating care in the home and in-patient settings. Nurses are available day and night to ensure the patient is comfortable.

Social workers provide counseling and support services, helping patients and their loved ones prepare for end-of-life decisions.

Chaplains provide spiritual guidance for patients and their loved ones.

Home health aides provide personal and homemaking services.

Physical, occupational and speech therapy services are available as needed.

Dieticians provide dietary consultations as needed.

Volunteers perform a wide variety of functions, including listening, reading books or running errands.
