After the form is
submitted, a Memorial Hermann representative will contact your company
to review protocol and process your specific service requests.
Completing the employer
protocol form establishes your company's policies and protocol for:
- Injury treatment
- Contact information
- Drug-testing
policies
- Billing instructions
- Specific follow-up
with designated company physicians
We will also contact
the occupational medicine coordinator at the hospital closest to your
workplace to make sure he or she is aware that your protocol has been
established and that it is understood.
