Memorial Hermann Cypress Hospital

When visiting the hospital for appointments, scheduled tests or delivery, please bring your insurance card and identification or you may fax a copy of the front and back to the Business Office at (346) 231-4154 .

After you have pre-registered, please review other steps to be completed as described below.

To Complete Your Registration

Once you have pre-registered using our online tool, you should also complete the following by the seventh month of pregnancy:

  • Review and sign consent forms in person at the Business Office.
  • If you are scheduled to have a c-section, you must have lab work done prior to your procedure. Call Pre-Admit Testing at (346) 231-4080 to set up an appointment.

Birth Certificate Information

If you would like to get started on your baby's birth certificate, you may fill out the form below and then bring it with you to Labor & Delivery when you have your baby.

If you are not married, the father's name may be completed on the certificate only if both parents sign the Acknowledgement of Paternity (AOP) form. Visit the Birth Certificate Clerk at Memorial Hermann Cypress Hospital to obtain the AOP form and instructions for completion.

  • Your first trimester of pregnancy is not too soon to begin preparing for your baby's birth. During this time, you may wish to participate in our maternity tour.
  • The  Infant Security guidelines provide good, sound parenting techniques that can also help prevent abduction of infants while in the hospital where your child is born and once you take the child home.
