Memorial Hermann invites all members of the community to register for the COVID-19 vaccine at a Memorial Hermann vaccination clinic.

Registered individuals who meet eligibility for vaccination will be notified when appointments are available. Memorial Hermann will make appointments available based on vaccine supply and will prioritize vaccine distribution in accordance with guidance from the Texas Department of State Health Services.

Please read below to learn if you are currently eligible to receive vaccination at Memorial Hermann and to review the vaccination registration process.

All members of the community 12 years of age and above may complete Memorial Hermann’s COVID-19 Vaccination Request Form (those age 12 and older for the Pfizer vaccine, and those age 18 and older for the Moderna and Johnson & Johnson vaccines). Registered individuals will be notified when vaccination appointments are available for them.

Yes. If you received your first dose outside of Memorial Hermann and are unable to receive your second dose at that same location, please complete Memorial Hermann’s COVID-19 Vaccination Request Form.

Yes, individuals who receive an invitation to schedule vaccination at Memorial Hermann can select a location for their appointment based on the vaccine being administered at that clinic, which will be noted next to the clinic name. Also, as Memorial Hermann begins to offer more walk-in vaccination opportunities, individuals will be informed upon arrival which brand of vaccine is available and can decide whether they wish to receive it.

Once you complete Memorial Hermann’s COVID-19 Vaccination Request Form, Memorial Hermann will notify you when appointments are available. Memorial Hermann will make appointments available based on vaccine supply and will prioritize vaccine distribution in accordance with guidance from the Texas Department of State Health Services. A link to schedule your vaccination will be sent to you via text message and/or email based on the communication preferences you enter when completing the request form.

No. Individuals only need to complete the request form once and will be notified when appointments are available. Completing the form multiple times will not affect how soon you receive an appointment.

When available, appointments will be filled on a first-come, first-served basis. If you cannot schedule your vaccination due to appointments being full or for any other reason, your name will be added back to the waitlist. You will be invited three times before being removed from the waitlist.

Completing the Request Form and Scheduling an Appointment After Receiving an Invitation

Unfortunately, we are unable to correct mistakes and typos in the request form. If you believe you have made an error, please complete a new request form with the correct information.

Yes. You may enter your email address and/or mobile phone on behalf of your loved one. However, please ensure all other information entered applies to your loved one, including first name and last name (as they appear on his or her photo ID), date of birth and eligibility criteria, if applicable.

After receiving an invitation to schedule your appointment and successfully scheduling vaccination, you will receive an automated confirmation text message from (281) 214-6637, which should arrive immediately, or by email from MHVaccination@memorialhermann.org, which typically arrives within minutes but may take up to an hour to arrive depending on your email service provider.

When you scheduled your vaccination, you should have received an automated confirmation text message from (281) 214-6637 or email from MHVaccination@memorialhermann.org. If you cannot find your confirmation, please call Memorial Hermann’s COVID-19 Vaccine Helpline at (833) 772-2864.

We strongly encourage you to keep your current appointment date and time. However, we understand circumstances can change. If you need to reschedule your appointment, locate the appointment confirmation that was sent to you via email from MHVaccination@memorialhermann.org and click on “Cancel My Appointment.” You will be able to reschedule after your initial appointment has been cancelled.

Memorial Hermann will automatically schedule your second dose at the time you receive your first dose based on the manufacturer’s guidelines. No action is needed on your part.

When you receive an invitation to schedule your vaccination appointment, you will be provided with the option to remove your name from the waitlist. In the meantime, no action is needed on your part.
