American Diabetes AssociationMemorial Hermann Southeast Hospital has been Recognized by the American Diabetes Association for meeting the National Quality Standards for Diabetes Self-Management Education.

Our program is for people with all types of diabetes, including type 1 diabetes, type 2 diabetes and gestational diabetes, and both newly diagnosed and well-established patients that want to improve their blood glucose control.

An individualized approach, which may include a group learning session, allows the patient to identify the behavior changes and strategies needed to control diabetes. The Diabetes Education Program is staffed by nurses and dietitians specializing in diabetes management.

A Physician's Referral is required. Instruction in Spanish language is available upon request.

To make an appointment for diabetes education, please call (281) 929-4112 Monday through Friday from 8 a.m. until 5:00 p.m. Patients are welcome and encouraged to bring family members to appointments.

Additional Support

Memorial Hermann Southeast will offer a Diabetes Support Group for all those affected by diabetes mellitus. The group will meet quarterly in the evenings. Virtual support groups are available monthly via Zoom from 6 p.m. - 8 p.m, click here to for dates and registration information.
