The Physicians at Sugar Creek Primary Care take their time to listen to each patient’s concerns and collaborate with them to help determine the right treatment plan. In addition, because immediate access to medical care is important, Sugar Creek Primary Care also offers convenient urgent care hours, on evenings and Saturdays.

All pediatric patients are required to be up-to-date on all immunizations or scheduled to receive recommended immunizations when visiting Memorial Hermann Medical Group practice locations unless there is a medical reason that the child cannot be completely vaccinated.
In accordance with the immunization schedule published by the American Academy of Pediatrics (AAP) and the Advisory Committee on Immunization Practices (ACIP) and the Centers for Disease Control & Prevention (CSC), the pediatricians of Memorial Hermann Medical Group recommend all children receive vaccinations for the safety and wellbeing of each child and the community.
We believe in practicing evidence based medicine and providing high quality health care for the children in our community. Based on all available literature, evidence, and current studies, we believe it is safe and appropriate to administer the vaccines according to the above schedule.
Physicians at Sugar Creek was the first clinic in Texas to be recognized as a Medical Home in 2008. The patient-centered medical home is a way of organizing primary care that emphasizes care coordination and communication to transform primary care into "what patients want it to be." Medical homes can lead to higher quality and lower costs, and can improve patients' and providers' experience of care.
NCQA developed the Diabetes Recognition Program (DRP) to provide clinicians with tools to support the delivery and recognition of consistent high quality care. This voluntary program is designed to recognize clinicians who use evidence-based measures and provide excellent care to their patients with diabetes.
Asthma is a major cause of illness with increasing prevalence in both children and adults. Over 20 million Americans suffer from asthma (with many more suspected undiagnosed cases) with annual treatment costs estimated at over $3.2billion as well as 14 million lost work and school days every year.
Millions of Americans have some form of diabetes, and millions more cases are undiagnosed. To encourage more effective treatment and management, BTE worked with diabetes experts to develop measures to assess and reward superior clinician performance.

Please arrive 30 minutes prior to your appointment time.
Please arrive 15 minutes prior to your appointment time to review your current information.
If you arrive late for your appointment, you may be asked to reschedule.
Payment is requested at the time service is rendered unless other arrangements have been made. We accept cash, check and all major credit cards. We will be happy to file insurance claims, but co-pays and deductibles that have not been met are due at time of service. Please be prepared to show your insurance card EACH visit.
If you need an existing prescription refilled, please contact your pharmacy. Once we have received the request, we will complete it within two business days.
Our policy is to communicate all test results to you, whether normal or abnormal. You will hear from us either in writing, by phone or by email. It is your choice. Please allow seven business days for your test results to be received and conveyed to you. You will be notified immediately by your physician if any of your test results are significantly abnormal. If you have not heard from your physician within seven business days, please contact our office.
Our physicians are able to provide the vast majority of your care in our office. Some insurance plans require that you have your referral approved prior to seeing another specialist or having a test performed. We will help you obtain this referral, but please allow two weeks for this process. When you contact us to request the referral, please include the specialty or specialist to whom you need the referral and the reason for the referral. We will notify you in writing when the referral has been approved by your insurance. Please note that many health plans will only authorize referrals to in-network providers. Should you wish to see a provider who is not in network for your plan, you will need to contact your plan's customer service representative.
