Improving the Health of Our Community

Memorial Hermann Community Resource Centers are open Monday through Friday, from 8 a.m. to 4:30 p.m. Closed daily from 12 p.m. to 1 p.m. for lunch. Located near several Memorial Hermann hospitals, Memorial Hermann Community Resource Centers provide convenient locations for individuals and families to get help signing up for health care, social services and health education. The goal of the Centers is to help you improve your overall health and well-being, which can lead to improved quality of life.

We help connect individuals and families to state and county medical benefits to support their health and well-being. Our team can assist with applications for medical coverage and SNAP benefits, provide referrals to community food pantries and offer access to onsite pantries for those facing immediate food needs. In addition, we help community members find utility assistance, legal services, financial resources and other essential support programs.

  • Completing Texas Benefits applications, including SNAP, Medicaid, CHIP, CHIP Perinatal and Medicare Savings Plans
  • Applying for Harris Health Financial Assistance

Linkage to Primary Health Care Services

  • Obtaining referrals to primary and specialty care providers
  • Meeting eligibility requirements and scheduling appointments

Health Literacy

  • Learning about health, wellness and social services
  • Managing chronic diseases through educational opportunities and self-management workshops

Food Resources

  • Acquiring food from our onsite emergency food pantry
  • Finding community pantries
  • Completing SNAP applications 

Social Service Resources/Referrals

  • Meeting basic needs
  • Attaining behavioral health resources

Frequently Asked Questions

All community members and patients of Memorial Hermann are eligible to receive services from the CRC.

We can assist with Harris Health Financial Assistance Program, state benefit programs such as Medicaid, SNAP, CHIP and Medicare Savings Plan. We also have a food pantry on-site and can provide linkage to medical care and social services.

Most eligibility applications will require proof of income, address and a photo ID, but please call for more information.
